The 4 Mandatory Items to Bring
3 Copies of Live Scan Form
Bring 3 copies of your official Request for Live Scan Service (Form BCIA 8016 in California or state equivalent). Ensure the top section has the ORI code, Job Title, and Agency Address pre-filled by the organization requesting your check.

• 1 Copy for your personal records (with ATI/TCN)
Valid, Unexpired Government ID
State and federal law requires verification of your identity with an unexpired government photo ID. Expired licenses, photocopies, and temporary paper printouts without official stamps are not accepted.

• Permanent Resident Alien Card (Green Card)
Payment Method (Check Ahead)
Be prepared to pay the local rolling fee plus government fees (if not pre-billed by employer). Rolling fees typically range from $20 to $45.
• Most private centers accept Cards / Apple Pay
• Police / Sheriff departments often require exact cash or money orders
• Call ahead to verify accepted forms of payment
Previous Rejection Notice (If Rescan)
If you are getting fingerprinted due to a previous rejection from DOJ or FBI, you must bring the official state rejection letter and your original receipt with the previous ATI number.
• State DOJ does not charge a second processing fee if the rejection letter is submitted.
What if you don't have a Primary Driver's License or Passport?
If you do not possess an active primary government-issued photo ID, some states allow identity verification through two (2) secondary documents, provided one has a photo and is accompanied by certified supporting records.
Frequently Asked Questions
Can a proctor fill out the top part of the form for me?▾
No. By law, the proctor cannot enter agency codes, ORI numbers, or Mail Codes. Only the agency that asked you to get fingerprinted can provide these details.
What happens after I get fingerprinted?▾
The technician hands you back your applicant copy of the form. Look at the bottom of the page — your unique ATI (Automated Transaction Identifier) will be handwritten or stamped on it. You will use this number to check your background check status online.
Should I wash my hands or use lotion before the appointment?▾
Wash your hands with soap and water to remove oils, but DO NOT apply greasy hand lotion right before your appointment. Heavy lotion can cause optical distortion and ridge blurring on the electronic scanner prism.
